Could aligned teeth be possible even without orthodontic treatment?
Hello, I’m Kim Sun-min, the chief director of Ceramic Dental Clinic. When teeth are crooked, the first treatment that usually comes to mind is orthodontics. However, in reality, many people find the long treatment period of one to two years burdensome. For people who work and live socially active lives in particular, wearing appliances for a long time can itself be a major concern.
So today, I’d like to introduce the case of a 27-year-old female patient who had been told at another clinic that “laminates won’t work.” Although she had severe crowding and crookedness, her alignment was improved using only laminates and crowns, without orthodontic treatment. I hope this case helps show that even when the teeth are this misaligned, a certain degree of improvement is possible without braces.

Condition before treatment
She was a 27-year-old female patient, and both the upper and lower front teeth were severely crowded. The two upper front teeth (the two large central incisors) were protruding forward, and a gap between them was also observed.

Initial facial photo – the upper central incisors are protruding, and spaces between the teeth are visible.
In particular, the upper lateral incisors (the teeth right next to the central incisors) had been pushed inward, which made the central incisors appear even more prominent. The lower front teeth were also protruding forward and overlapping each other.

Before treatment – severe overlap and mismatched tooth angulation are visible in the upper and lower front teeth.

Before treatment - if prosthetics are made without reducing them as they are, interference between the upper and lower teeth is unavoidable.

The patient’s biggest wish was, “I want to straighten my crooked teeth, but I don’t want orthodontics because it takes too long.” Since she had already been told at another clinic that laminates would not be possible, she was also very worried about whether treatment could really be done.
Why laminates were possible even for severely crooked teeth - Treatment plan
When the degree of crookedness is severe, it is generally often judged that “laminates alone would be difficult.” This is not an incorrect statement. If every tooth is approached in the same way, it often becomes truly difficult.
However, the key is to adjust the amount of reduction differently for each tooth. Depending on how much each tooth’s angle is deviated, a different approach can allow sufficient improvement even without orthodontics.
The treatment plan for this case is summarized as follows.
| Category | Teeth involved | Approach | Amount of reduction |
|---|
| Teeth with severe angulation deviation | 2 upper lateral incisors | Crowns | Sufficient reduction required |
| Remaining teeth | Other teeth | No-prep / minimal-prep laminates | Minimal reduction |
In other words, instead of heavily reducing all teeth in the same way, we established a strategy in which only the teeth with severe angulation were reduced sufficiently, while the rest were preserved as much as possible. The patient understood this well and agreed.
The tooth shade was chosen to be natural yet bright, and since the patient’s main concern was improving the alignment of the upper front teeth, treatment focused on that area.
Before-and-after treatment results
How much improvement was possible?

The teeth with severely misaligned angles were reshaped and restored with laminates and crowns, while the remaining teeth were treated with laminates using only minimal reduction, achieving both tooth preservation and aesthetic harmony.

Neatly aligned upper front teeth - the most important area aesthetically
The lower front teeth also harmonize naturally with the upper teeth, and the gum line remains healthy and stable.

After treatment - the lower front teeth were also arranged beautifully.
After the procedure – the shape and shade of the lower front teeth blend naturally with the upper teeth, and the gums are in a healthy condition.
If you compare the before-and-after results, you can see that this level of alignment improvement is possible even without orthodontics. With a natural yet bright shade, the overall aesthetic quality was enhanced, and a functionally stable result was achieved.
What to note in this case
To summarize this case:
- Different doctors may have different judgments about whether laminates are possible.
- Especially for severely crooked teeth, there may be doctors who determine that treatment with laminates is possible after sufficient discussion with the patient.
- The key is to adjust the amount of reduction differently for each tooth.
- Only the teeth with severe angulation deviation were reduced sufficiently, while the rest were treated with minimal reduction to minimize unnecessary tooth damage.
- Laminates can be a good alternative for people who find the long duration of orthodontics burdensome.
- However, laminates are not possible for every case. A precise diagnosis that comprehensively analyzes the position of the teeth, their angulation, and the bite condition must come first.
You can regain a neat smile without orthodontics
For those who are concerned about tooth alignment but find orthodontics burdensome, laminates can certainly be a good option. Of course, what matters most is an accurate diagnosis and a personalized treatment plan.
To achieve the best result, it is necessary to comprehensively analyze the patient’s facial shape, smile line, degree of gum display, and how the upper and lower teeth fit together. It is not simply a matter of saying, “We’ll do laminates,” but rather carefully planning why one tooth should be reduced this much and another only minimally. That detailed process makes the difference in the result.
