Is the order important when getting implant treatment?
Among the patients who come to the dental clinic for treatment,
there are actually quite a few like this.
It is a case where both the front teeth and the molars have problems,
but it is not a full-mouth implant case.
In that situation, they start to wonder where treatment should begin
and in what order it would be best to proceed.

It can vary depending on the case.
In general, it is more appropriate for bite alignment
to do the molars first
and then proceed with the front teeth.
A female patient in her 40s.
Her first visit was around the time when the summer heat was fading
and autumn was beginning to arrive.
When she first came in, several molars were already missing.

First, we took a panoramic X-ray, periapical X-rays,
and a CT scan to check everything carefully.
Looking closely, there was quite a lot of calculus between the teeth
and in the interdental spaces.
As a result, the health of the gums was of course
not in good condition.

Overall, this was not a case where oral care
could be said to have been well maintained.
The front teeth were also a little loose and not in good condition, but
the patient also wanted to improve the appearance of her teeth aesthetically,
so we decided to attempt prosthetic treatment.
In cases like this, the first thing to consider is
the bite between the teeth, as mentioned above.
Bite alignment is a very important part, and in cases like this,
if you take occlusion into account, the order of implant treatment
and front tooth treatment should begin with the molars.
First, we performed scaling to remove the calculus.
Tooth extraction and implant surgery were performed.
First, implant placement in the lower jaw was carried out.

This was done about 10 days after the first visit.

After that, the upper jaw was implanted about a month later.
Depending on the situation, we also consider whether to do the upper or lower side first,
but in this patient, since teeth were missing on both sides,
we proceeded first with the lower molar surgery, where prosthetic restoration could be placed relatively quickly,
so that she could start eating on at least one side sooner.
While waiting for the temporary teeth to be made,
they are used until the final prosthetic is ready,
and during this process we check the shape and use it as a reference
for making the final prosthetic.

Front tooth root canal treatment
For the front teeth, we were planning to proceed
with prosthetic treatment.
However, after the first prep, the patient complained of tooth sensitivity
and discomfort,
so we decided to perform root canal treatment.


The reason was that the patient reported pain in the front teeth.
Because the roots had already been very short from the first visit,
we decided to proceed with root canal treatment.


This is also called endodontic treatment.
After removing the infected pulp,
cleaning the inside of the tooth thoroughly,
and then disinfecting and sealing the empty internal space
with clean, sterilized materials, that is the treatment method.
Final front tooth prosthetic
For the final prosthetic, a zirconia crown was chosen
and the plan proceeded.
Taking into account the tooth color and translucency of the zirconia crown,
as well as every line of the gum contour,
the design was carried out so that the difference from natural teeth would be minimal.
The patient said she did not have any major complaints about the temporary teeth,
but I remember her saying that she liked the finished teeth even more.
Implant treatment: results of molar and front tooth prosthetic treatment
This was a long-term treatment with a fairly long treatment period.
It took quite a while, but the treatment ended successfully
with results that satisfied the patient.
When there are problems in both the molars and the front teeth,
it is important not to treat everything indiscriminately,
but also to decide how the treatment should be done.

You must be sure to check the bite and carry out treatment accordingly,
and this was a case where we also had to carefully examine whether there were any other problems with the teeth themselves.
In the case of the front teeth, this was not part of the original treatment plan,
but during treatment the patient complained of pain in the front tooth area,
so after checking, we decided to perform root canal treatment.
Since the tooth roots were already very short from the initial visit,
it seems the patient came at an appropriate time and was able to complete treatment well in sequence
along with the molars.
