The words I hear most often
in the exam room
When I see patients, I often hear people in their late 30s to early 50s say, "I feel like my face suddenly sagged."
A noticeably changed facial contour, deeper nasolabial folds, and a less defined jawline do not appear overnight.
Today, I’d like to talk about what causes skin sagging and how the everyday habits we repeat without thinking can speed up skin aging.
Why skin sags,
looked at anatomically
Skin elasticity is maintained by the collagen and elastin fibers in the dermis.
After the mid-20s, the rate of collagen synthesis gradually begins to decline, and after the 40s, that decrease becomes even more pronounced.
As facial fat decreases and the skeletal structure itself changes, the structures that support the skin become weaker overall.
This is the most fundamental physiological background behind skin sagging.
When poor daily habits are added on top of this, aging can progress much faster.

The first habit that lowers elasticity — neglecting UV protection
Ultraviolet rays are the strongest external factor that accelerates skin aging.
UVA penetrates deep into the dermis, activates collagen-degrading enzymes, and damages elastin fibers.
Even on cloudy days or indoors, UVA passes through glass and affects the skin.
Using a sunscreen with SPF 50 or higher and PA+++ or higher, and reapplying it every 2 to 3 hours, plays an important role in reducing the causes of skin sagging.
It is best to use UV protection every day, regardless of the weather.

The second and third habits — lack of sleep and rapid weight changes
During sleep, growth hormone is secreted and skin cell recovery takes place.
If sleep is chronically insufficient, cortisol levels rise, which inhibits collagen synthesis.
As blood circulation declines, the supply of oxygen and nutrients to the skin becomes less smooth.
Rapid weight loss from dieting can cause facial fat to disappear in a short period of time, reducing the volume that supports the skin.
The skin cannot keep up with that speed, and in some cases remains loose, which acts as one of the causes of skin sagging.
A regular sleep pattern and gradual weight management are more helpful than expected in maintaining skin elasticity.

How Ulthera Prime and Thermage approach sagging skin
When it is difficult to feel a change with everyday care alone,
we recommend Ulthera Prime and Thermage depending on the degree of skin sagging and skin type.
Ulthera Prime uses high-intensity focused ultrasound (HIFU) technology to deliver energy to the muscle and fascia layer (SMAS layer) without touching the skin surface.
Like focusing sunlight to a single point with a magnifying glass, it concentrates heat energy only on a specific deep layer of the skin and is known to help collagen formation.
It may be helpful for people whose sagging has progressed relatively clearly, or for those concerned about the jawline or cheek sagging.
Thermage uses radiofrequency (RF) energy to apply uniform heat to the dermis.
As the heat is delivered deep into the skin, collagen fibers contract, and then new collagen is gradually produced.
It may be helpful for people with overall reduced elasticity and rough skin texture, or for those whose skin volume has decreased.
With both methods, positive changes in the skin often appear gradually over 4 to 12 weeks.
Because the depth reached by ultrasound and the depth at which radiofrequency acts are different, they can also be used together in a complementary way.

Post-treatment care: how to maintain the effect for longer
For one week after treatment, it is best to avoid activities that significantly raise body temperature, such as saunas, jjimjilbangs, and high-intensity exercise.
This is because the skin that has received heat stimulation needs time to recover stably.
Please continue to manage sunscreen consistently, and adequate water intake and moisturizing care help support the environment for collagen formation.
When skin boosters such as Rejuran and Juvelook are combined with lifting procedures, they can help with elasticity, hydration, and skin texture at the same time, so you may expect synergy.

Honest treatment for a comfortable face
I always say this in the exam room.
Every skin is different, and even with the same method, there are people for whom it matches their current skin condition and people for whom it does not.
The causes of skin sagging are not singular, and the pattern of aging also appears differently from person to person.
That is why, rather than following trends without an accurate diagnosis, it is important to first identify what approach is suitable for your current skin condition.
